Nuclear Throne goes extremely hard. It's a classic grandfather roguelike for sure, but it's way more arcade-y than any other of the thousand roguelikes I've played, due to these differences:
-> difficulty is increased + full runs are shorter + player deaths are often instantaneous

And "build refining" aspects are minimal: You only choose some perks (things like max hp, ammo, or pickup range), and you grab whatever guns seem to work well at the time. This means there are neither "op god runs" nor "♥♥♥♥♥♥ RNG losses".
There's also no real meta-progression so you will slam that R key until you practiced enough out to reach the titular throne. And once you finally sit on it, you realize how much harder things get from there.

It's over 10 years old, but holds up super well in 2025. Edit: In fact, yesterday they even dropped a major content update along with improvements such as supporting widescreen and modern refresh rates!

And then there's the online multiplayer mod NTT, it has steam integration and playing this game coop is absolutely deranged in a good way. Strong recommendation both for solo and coop play.
